My name is Asif. I hold my account no ———–in HBL UET Lahore branch. Recently, I have found a mistake in my date of birth as I checked my account details from bank. Bank has mistakenly put my wrong date of birth of 24 September 1991, which is contrary to my actual date of birth. I request the manager of concerned branch that he should direct his staff to amend my date of birth which should be 11 September 1991 as per my document.

Wrong Information on Credit Card Application

Dear Manager,
Last week, I came to the bank, and wrote an application for a credit card. As the credit card was of my father’s bank account, and he could not come due to some health issues so I came in his place to fill the application form.
However, today my father received an email that his application for a credit card is in process, and he noticed that the CNIC number on the application is incorrect. I am sorry for this inconvenience but please correct the information on the form to avoid any sort of misunderstandings in the future. I have attached the copy of the form along with the corrections.
